This is a small tortuous device consisting of a steel ring that sits around the nipple with a steel frame that houses a cloverleaf nipple clamp and screwing device. Once fitted, it is the tension on the nipple being pulled that holds it in place, while slowly turning the dial will pull and stretch the nipple to achieve the desired effect. The clamps are removable, allowing you to use your own favorite clamps or similar ones.
